Equipment Warranty
- The nebulizer has an automatic 5 year warranty from the date of manufacture.
- If you have any issues with the equipment, do not throw it away, contact our office for troubleshooting and warranty replacement.
Financial Responsibility
Deductibles and Co-Insurances Apply – Based on your Insurance Provider
North Carolina Medicaid and NC Medicaid Managed Care Organizations – Amerihealth, Healthy Blue, Carolina Complete Health, United Healthcare of North Carolina, or Wellcare
- The nebulizer is covered at 100% as long as the patient has full medical coverage.
- Patients are allowed one nebulizer every three years.
Private Insurance
- Active Healthcare is in network with many private insurance companies.
- Equipment cost will depend on your benefits; deductibles and coinsurance apply.
- Contact your insurance company with questions about your benefits.
Private Pay – Cash Price
- Call our office at 919-870-8600 Ext 83 ASAP to qualify for our discounted cash price.
- Payment is required within 48 hours of equipment receipt.
Tricare and Medicare – Rent to Purchase
- Nebulizer is processed as a rental over 13 months
- Equipment cost will depend on your benefits; deductibles and coinsurance apply.
